If you wake up with a sore jaw, hear clicking or popping when you chew, or get frequent tension headaches, the cause may be your jaw joint — the TMJ. At our Bellevue office, Dr. Bauer treats TMJ disorders with a conservative, step-by-step approach, and for the right patients, therapeutic Xeomin can be a remarkably effective way to relieve the muscle tension behind the pain.

Here's how we think about jaw pain and where Xeomin fits in.

What Is TMJ Disorder?

TMJ stands for the temporomandibular joint — the hinge that connects your jaw to your skull. When that joint and the muscles around it are overworked or out of balance, the result is called a TMJ disorder, or TMD.

Common signs include jaw pain or tightness, clicking or popping when you open and close, difficulty chewing, headaches, and even tension in the neck and shoulders. Many people don't realize their headaches or worn-down teeth trace back to their jaw.

What Causes Jaw Pain and Clenching?

A lot of TMJ trouble comes down to overactive jaw muscles. Clenching and grinding — often during sleep and often stress-related — keep those muscles in a constant state of strain.

Over time that tension can wear down teeth, strain the joint, and trigger chronic soreness and headaches. The goal of treatment isn't just to mask the pain, but to calm the muscles and protect your teeth.

Our Conservative, Step-by-Step Approach

We treat every case individually and start with the least invasive option that fits the problem. For TMJ, that usually means working through these tools in order:

  1. Night Guards and Splints For many patients, a custom night guard is the first line of defense. It cushions the teeth, reduces the damage from grinding, and can ease pressure on the joint while you sleep.

  2. Bite Adjustment Sometimes the way your teeth come together is part of the problem. A small, targeted bite adjustment can help your jaw close more evenly, reducing the strain that drives clenching and discomfort.

  3. Looking at the Airway Sometimes the real driver isn't the jaw itself — it's the airway. Grinding and clenching during sleep can be the body's response to a breathing problem, including sleep apnea. When we suspect that breathing may be behind your bruxism, we may recommend a sleep study and, if appropriate, treatment aimed at the airway. Addressing the underlying cause often does more for your jaw than treating the symptoms alone.

  4. Therapeutic Xeomin When muscle tension is the main driver and cannot be overcome with non invasive treatment, Xeomin can be a highly effective option. Dr. Bauer administers therapeutic Xeomin directly into the overactive jaw muscles, which relaxes them and reduces the force of clenching and grinding. We specifically use Xeomin because it's a highly purified neurotoxin — sometimes called a "naked" injectable because it doesn't carry the extra surrounding proteins found in some other products. That purity matters: those added proteins are part of why some patients build up a tolerance over time, and Xeomin's formulation lowers that risk, so it tends to keep working well treatment after treatment. The result for many patients is less jaw pain, fewer tension headaches, and relief that conventional approaches alone didn't fully provide.

How Does Xeomin for TMJ Work?

Xeomin temporarily relaxes the specific muscles it's injected into. For TMJ, Dr. Bauer targets the large masseter muscles responsible for clenching, calming them so they can't contract as forcefully.

The treatment is quick, done right in our office, and requires no downtime. Effects typically build over a few days and last several months, at which point the treatment can be repeated if it's helping.

Is TMJ Xeomin Right for Me?

Xeomin isn't the starting point for everyone. We consider it when:

  • Muscle tension and clenching are clearly driving your symptoms

  • Night guards or bite adjustments alone haven't fully resolved the pain

  • You're dealing with persistent tension headaches linked to your jaw

The best way to know is a conversation and an exam. We'll look at your bite, your jaw function, and your symptoms together, and recommend the most conservative path that's likely to work for you.

What Makes Our Approach Different

We don't jump straight to the most aggressive option. Dr. Bauer evaluates each patient individually and works through conservative steps — guards, bite adjustment, and Xeomin — to find what actually addresses the cause of your jaw pain rather than just the symptoms. Because the Xeomin is administered in-house by Dr. Bauer as part of your overall dental care, your jaw, bite, and teeth are all considered together rather than in isolation. We also chose Xeomin deliberately for its purity and lower tolerance risk, so it keeps working over the long haul. (We're happy to discuss cosmetic uses if you ask, but our focus here is therapeutic relief.)

If jaw pain or clenching has been wearing you down, you don't have to just live with it. We treat TMJ as part of our comprehensive dentistry services, and we'll help you find relief at a pace you're comfortable with.

Frequently Asked Questions

Does Xeomin for TMJ hurt? The injections are quick and most patients describe only a brief pinch. There's no numbing required and no downtime afterward — you can return to your day right away.

How long does TMJ Xeomin last? Results typically build over a few days and last several months. Most people see the benefits .

Why do you use Xeomin instead of other injectables? Xeomin is a highly purified neurotoxin without the extra proteins some other products carry. That purity is associated with a lower chance of building up a tolerance over time, which means it tends to stay effective treatment after treatment.

Will Xeomin change how my face looks? Therapeutic Xeomin for TMJ targets the jaw muscles involved in clenching, not the muscles treated for cosmetic wrinkles. As the masseter muscles relax, some patients notice a subtly slimmer jawline, but the goal is pain relief and reduced clenching.

Can grinding my teeth be related to sleep apnea? Yes. For some people, nighttime grinding and clenching are connected to a breathing problem during sleep, such as sleep apnea. If we suspect that may be the case, we may recommend a sleep study and treatment focused on the airway, since addressing the root cause can do more for your jaw than treating the symptoms alone.

Do I have to start with Xeomin? No. We usually begin with the most conservative options — a night guard and, when appropriate, a bite adjustment — and consider Xeomin when muscle tension is the main driver or other steps haven't fully resolved your symptoms.

Can Xeomin replace my night guard? Not necessarily. The two can work together — a guard protects your teeth from grinding while Xeomin reduces the muscle force behind it. Dr. Bauer will recommend the right combination for your situation.
