Are same-day crowns made from the same materials?

Yes, and this surprises people. The ceramic blocks we mill in the office are lithium disilicate and zirconia from the same manufacturers that supply dental labs. The ceramic doesn't know where it was milled.

A lab crown isn't made of something stronger or fancier. In most cases, it's the same class of material, shaped by a different machine in a different building.

Do same-day crowns fit as well as lab crowns?

The traditional lab process has more steps where small errors creep in: impression material can pull or distort on the way out of your mouth, the poured model can vary, and the crown is made against that model, not against your actual tooth.

A same-day crown starts with a direct digital scan of the prepared tooth, and the mill carves to that data with fraction-of-a-millimeter precision. Published research on chairside CAD/CAM crowns shows marginal fit within the same clinical standards as lab work. In practice, I check and adjust every crown against your bite either way. The standard doesn't change based on where the crown was made.

One bonus: your crown's design file is saved. If anything ever happens to it years from now, the remake starts from data, not from scratch.

Do same-day crowns last as long?

Chairside CAD/CAM restorations have been studied for a long time now. Peer-reviewed research has followed them past the ten-year mark with survival rates on par with lab-made crowns.

And here's the part that matters more than fabrication location: how long any crown lasts depends mostly on fit, bite adjustment, bonding technique, and how you care for it at home. A well-bonded, well-adjusted crown with good hygiene behind it is a long-term crown, whether it was milled fifteen feet from your chair or fifteen miles away.

What does same-day bonding actually do for you?

This is the underrated advantage. With a lab crown, your freshly shaped tooth spends two to three weeks under a plastic temporary. Temporaries can leak, loosen at the worst possible moment, and leave teeth sensitive.

With a same-day crown, the tooth is sealed under its permanent crown within hours of being shaped. Less post-op sensitivity, no temporary to babysit through a vacation, one round of numbing, and one visit off work instead of two.

When do we still choose a lab crown?

Here's the honest part. There are cases where a lab is the better call, most often a single front tooth, where matching the color, translucency, and character of the natural tooth right beside it takes a ceramist layering porcelain by hand. Certain complex multi-tooth cosmetic cases and some implant-supported designs also earn a lab's involvement.
